Financial Policy

Our goal is to provide outstanding comprehensive, personalized care that is complemented by outstanding customer service from our support team–from the front desk receptionist to our billing specialist. We work to support our patients with their financial responsibilities in conjunction with their personal therapy goals. We contact each patient’s insurance company to determine what his or her outpatient physical therapy benefits are. We will inform you of your benefits, how we will process your claims and your financial responsibilities.

Insurance Information

As a courtesy to all of our patients, we will submit claims to your primary and secondary insurance carriers. If there is a balance left owing, such as a deductible or co-insurance, we will send you a statement at the end of each month. For any balance past due (after 120 days from the date billed), there will be a 1.5% finance charge assessed each month.

Some of the insurance companies that we contract with are:

  • Premera Blue Cross
  • Group Health NW
  • Uniform Medical
  • Medicare
  • First Choice
  • Asuris NW
  • Worker’s Comp

It is impossible for us to know all the different policies for each individual insurance company, but we will try to the best of our ability to inform you of your outpatient physical therapy benefits and to keep you informed of any limitations of services.

Are you without insurance coverage?

No problem, we offer a cash option for those patient that have no medical insurance coverage. No insurance claims will be sent and patients pay at the time of service, unless other arrangements are made in advance.

Motor Vehicle Accident Claims

In case of a motor vehicle accident, we will only bill your personal motor vehicle insurance company. We do not bill third party claims. If it is a third party claim, we ask that you provide us with your personal medical insurance information or pay each visit at the time of service. It is not appropriate to ask that your therapist wait until you settle (usually 1-2 years) in order for payment to be received. Any outstanding claims will be assessed a finance charge of 1.5% after 90 days.
